How Much Does Therapy Cost?

I charge as a “fee for service”, meaning payment for service is due at each session.  I accept cash, check, Venmo, FSA/HSA and credit cards for your convenience.  If you have questions about my rates and availability, please call me at 808-352-6850 or email me at ampmft@ampfamilytherapy.com.

Insurance:

Please note as of June 1, 2019 I am no longer contracted with any insurance company.

If you think we are a good fit but I am out-of-network with your PPO insurance plan, I can provide you with a bill to submit for reimbursement or credit toward your deductible.

Here are some common questions about insurance.  Please contact your insurance carrier to help answer the following questions before contacting me for an appointment:

• Does my insurance plan include Behavioral Health benefits?
• Do I have a deductible and if so, how close am I to reaching my deductible?
• What is the coverage amount per therapy session?
• Are there mental health diagnoses that are excluded from my treatment coverage?
• Can I see Amy M. Price, LMFT as an out-of-network provider?
• If I choose to use AMP Family Therapy Inc./Amy M. Price, LMFT as an out-of-network provider, how is my copay, deductible, and reimbursement affected?

Payment Policies

Payment is due at the time of service. That payment includes your out-of-pocket payment that may be applied to your deductible, or the out-of-pocket amount for cash paying clients.

I strongly encourage all of my clients to be informed of their insurance policies, benefits and claims payment procedures prior to our first appointment so that you do not run into an unexpected financial obligation.
